Progressive Life Center, Inc. (PLC) provides culturally-sensitivity human services to children, youth and families in the District of Columbia and surrounding Maryland, throughout Greater Philadelphia, and in most of Delaware. PROGRAM OVERVIEW
The Home American Rescue Plan (ARP) program is a supportive housing services program designed to make homelessness brief and non-recurring. The services provided are to empower individuals by assisting, guiding, and supporting individuals to overcome challenges and improve their overall well-being. Culturally responsive case management, financial literacy education, employment assistance, and referrals to address participants’ unique needs, ensuring sustained housing stability are provided based on the individual’s needs.
POSITION RESPONSIBILITIES
- Provide relevant crisis intervention support, advocacy service and community resources and program referrals.
- Maintain and service a caseload of 15 – 24 clients and families.
- Conducting intake assessments and developing Individualized Service Plans (ISPs).
- Holding regular check-ins and tracking participant goals.
- Connecting clients to supportive services including domestic violence intervention, veteran’s services, and community resources.
- Providing health and wellness support: Medicaid/ACA enrollment, mental health referrals, and substance use disorder (SUD) treatment.
- Assisting with public benefits (TANF, SNAP, WIC, SSI, SSDI) and financial literacy via CRA-partnered banks.
- Supporting childcare referrals and transportation needs.
- Link clients to parenting classes via Nurturing Parenting® curriculum, life skills coaching, and community engagement.
- Documenting all services in HMIS and Mainstay; monitoring housing retention metrics.
- Participating in core training, trauma-informed care, and case management best practices.
- Carry, monitor and report all emergencies or after-hours calls to on-call phone at least one week per month.
- Complete monthly review reports with foster parents, clients and supervisors and submit to program director, prior to the end of the month.
- Provide case management and referral services; provide and link clients/biological families to follow-up services.
- Attend and participate in weekly supervision and case conferences.
- Attend and participate in interdisciplinary team meetings.
- Develop service plans, progress reports, termination summaries and follow-up documentation.
- Enter and maintain accurate data into database (e.g., FamCare, HMIS, Mainstay).
- Complete other duties as assigned.
- Sensitivity to and competence in the cultural differences that are present among service population.
- Ability to forge a mutually respectful partnership with persons served and their families in which they are helped to gain the skills and confidence to address any issues and problems they face.
- Conviction about the capacity of people to grow and change.
- Ability to work in partnership with other team members.
- Ability to set limits and maintain the helping role of the practitioner and to intervene appropriately to meet the needs of the persons served or other family members.
- Ability to drive vehicle to perform several of the essential functions of this position.
QUALIFICATIONS
- Bachelor’s or master’s degree in psychology, social work, counseling, education or other human service field from accredited college or university.
- Bilingual in English and Spanish.
- Minimum three (3) to five (5) years of progressive experience working with children, youth, and families in human services setting.
- Experience working with people experiencing homelessness or a shelter environment.
- Experience in best practices & training.
- Proficient MS Office Suite and databases skills.
- Attention to detail, conflict resolution, planning, and organization skills.
- Ability to successfully interact with internal and external stakeholders.
- Demonstrated ability and enthusiasm for working collaboratively.
- Evidence of effective verbal and written communication skills.
- Ability to work a flexible schedule to accommodate participant’s needs.
- Certification in NTU Service Delivery Model within one (1) year of hire.
- Certified in CPR/First Aid.
- Trauma informed care certification preferred.
- Valid driver’s license and reliable vehicle with the ability to travel within the City of Philadelphia and surrounding regions.

Salary will be commensurate with experience.
PHYSICAL DEMANDS

- Must be able to remain in a stationary position 50% of the time.
- The person in this position needs to occasionally move about inside the office to access file cabinets, office machinery, etc.

- Must be able to position self to stoop, kneel, crouch, or crawl.

- The person in this position frequently communicates with employees/customers/clients ; and must be able to exchange accurate information by perceiving the nature of sound, near and far vision, depth perception, providing oral and written information.
- Must be able to detect funnel clouds from long distances.
- Frequently move files weighing up to 20 pounds throughout the office space.
- Must be able to participate in fire drills or other drills during an emergency.
- Must be available on call and often respond to situations during off hours which can disrupt normal sleep patterns and cause fatigue.
- Must be able to face a significant amount of emotional stress which can cause related physical stress on muscles as well as other physical symptoms such as headaches and fatigue.
WORK ENVIRONMENT
- Normal office environment or residential facility, but also may travel locally to visit clients within their homes, meet with service providers, and/or attend meetings: schools, hospitals, mental health clinics, private practices, prisons, corporations, and numerous public and private agencies.
- This position is ineligible for telework and the incumbent must work on-site.
